PATH is seeking candidates with experience in social services to join our new Care Coordination Program in Orange County as a Program Manager.
ABOUT OUR PROGRAM
The Care Coordination and Encampment Resolution program offers care coordination and outreach, through a multi-disciplinary team, to unsheltered individuals and high utilizers of Orange County resources and community services. The goal of the program is to link unsheltered individuals to shelter, supportive service and housing by providing continuity of care from street to stabilized placement.
WHAT IS A PROGRAM MANAGER
As part of the Orange County team, the Program Manager will provide high level of expertise, guidance, and consultation in the areas of direct client contact, homeless program delivery and reporting, community relationships/partnership s and management of personnel. Responsibilities include:
- Program Management & Oversight-Provide oversight in collaboration with the Director and Associate Director to ensure successful implementation, coordination of participant services, achievement of program performance measures, and evaluation of services provided. Ensure that programs and projects are in compliance with contracts, partnership agreements, and all other funding requirements, program goals, and stakeholder expectations. Operate as the primary contact person for all program matters. This includes program monitoring by funders, responding to specific program inquiries from outside partners, and working with the Finance department to monitor program budgets.
- Staff Support/Development-Provide direct supervision to all staff and monitoring workload assignments and adjust work to be accomplished by staff; adjust program and project priorities in consultation with leadership; and prepare schedules for work completion. Conduct weekly supervision meetings with staff; provide regular and appropriate feedback including training, opportunities for professional growth, verbal and written improvement plans when needed, and regular evaluations.
- Program Outreach and care co-ordination–Educate, network and represent PATH programs to local agencies, participants, and other organizations. Seek opportunities for collaboration with other homeless service providers, non-homeless service providers, and the continuum of care.
- Reports/ Contracts and Compliance-Review and authorize all temporary financial assistance, subsidy program and contract reports prior to submittal. Evaluate line staff monthly reports to assemble full department reports. Review and explain contract scope and services to staff. Submit regular monthly report. Perform periodic internal audits. Attend all monitoring site visits.
- Performance–Develop strategies to ensure all program outcomes and goals are achieved.
WHAT YOU BRING
We’re looking for candidates that:
- Demonstrate proficiency with technology used for work outputs including Microsoft Office and electronic health record systems
- Have training in Motivational Interviewing, Trauma Informed Care, and other related techniques
- Display the ability to represent the interests of the agency and interact and build rapport and collaborative relationships with a diverse range of individuals
- Possess a high level of tolerance and understanding for individuals with urgent and multiple case management and health needs
- Work just as well independently as they do on a team
- Exercise mature judgement, and are highly motivated, self-starting and proactive
- Are excellent at communicating, whether in writing or verbally
- Have a strong sense of prioritization and can coordinate multiple demands in a high-pressure environment
PREFERRED QUALIFICATIONS
An excellent candidate for this role will:
- Bachelor’s Degree in a relevant field.
- HMIS Experience.
- Experience with program start-up.
- Supervisory experience in homeless services or related social services field strongly preferred.
- Training with advanced and evidence-based interventions including, but not limited to: Engagement and rapport building with hard-to-reach clients, Harm Reduction strategies, Motivational Interviewing, Housing First practices, Non-violent crisis intervention techniques, Critical Time Intervention
MINIMUM QUALIFICATIONS
To be considered for this position, we require all candidates:
- Two (2) years of experience in in healthcare, case management, homeless services or working with vulnerable populations.
- Experience directing the work of others in homeless services or related social services field with strong skills in staff supervision, program development and implementation and development of policies and procedures.
- Experience working with program funders.
- Experience analyzing program procedures and practices to identify barriers and impediments to achieving goals and making recommendations for course-correction.
MANDATORY REQUIREMENTS
Regardless of qualifications, candidates must:
- Have employment eligibility verification
- Have or be able and willing to obtain CPR/First Aid training
- Provide proof of full COVID-19 vaccination
- Successfully complete the following as a condition of hire:
- Tuberculosis Test
- Background Screening
- Drug Test
DRIVING
Driving is an essential function of this position. To meet the employment requirements for this role, all candidates must:
- Have reliable transportation
- A valid driver’s license
- Proof of insurance and ownership for personal vehicles used during work duties
- The ability to qualify for PATH's insurance coverage
COVID-19 VACCINATION
Effective 11/01/2021 PATH will require all employees to be fully vaccinated for COVID-19. All prospective hires will be expected to provide proof of full vaccination as part of the pre-employment credentialing process.
FLEXIBLE WORK
PATH is a California employer. Therefore, all regular and customary work must be performed within the state. Approval for work outside of the office, does not establish approval for work to be conducted outside of California.
