Job Description

Employment Type: Full-Time Hourly, 1.0 FTE (0.75 FTE Community Outreach & Partnerships / 0.25 FTE Direct Services)

Reports to: Program Director

Wage: $25-$27/hour, depending on experience

Job Location: Larimer County / Fort Collins, CO, with regular local and regional travel

ABOUT THE JACOB CENTER:

The Jacob Center offers trauma-responsive youth, family, and counseling services in Northern Colorado. Serving Northern Colorado since 1988, we provide services to children, teens, and families using Trust-Based Relational Intervention® (TBRI®), Positive Youth Development, and other evidence-based practices. Our mission: Working with youth, families, and the community to foster resilience, promote felt safety, and inspire positive and lasting change.

At The Jacob Center, we believe that healing happens in community. We're looking for a relationship-driven Community Outreach Coordinator to connect families with the resources, training, and support they need.

THE OPPORTUNITY:

This is a hybrid role for someone who wants both the reach of community-facing outreach work and the satisfaction of direct family support. You'll split your time between building community and family-facing partnerships (0.75 FTE) and providing hands-on, TBRI®-informed coaching to families and youth (0.25 FTE).

WHAT YOU'LL DO:

Community Outreach & Partnerships (0.75 FTE)

●      Recruit and enroll caregivers and families into TBRI® Caregiver Series trainings and other agency programs.

●      Coordinate program onboarding for families and community partners accessing TBRI®, ensuring timely follow-up and connection to services.

●      Build and maintain partnerships with schools, school districts, providers, and community organizations to support outreach, referrals, and resource-sharing.

●      Coordinate with the TBRI® team on outreach materials, scheduling, and enrollment logistics; serve as the interagency point person for TBRI®-related programming needs.

●      Maintain outreach and enrollment data and tracking records to support accurate reporting.

●      Represent The Jacob Center at community events, resource fairs, partner meetings, and fundraisers, in collaboration with the Development and Marketing team.

●      Collaborate with the Development team to support volunteer coordination and management.

Direct Services & Family Support, TBRI®-Informed (0.25 FTE)

●      Provide direct TBRI® coaching, strategies, and support to enrolled families and youth alongside programming staff, connecting families to resources and referrals identified through outreach.

●      Help ensure families who are recruited and referred are actually connected to services, closing the loop between outreach and lived experience of care.

●      Apply TBRI® principles from the Caregiver Series in all family-facing interactions.

LOCATION AND SCHEDULE:

This position is based in Larimer County, with regular travel to schools, partner organizations, and community sites. Expect a full-time (approximately 40 hours/week) schedule with occasional evenings and weekends for community events and family scheduling needs.

REQUIRED QUALIFICATIONS:

●      Bachelor’s degree and 2+ years of related experience in community outreach, case management, family support, or human services.

●      Strong organizational and communication skills, with comfort using data entry and tracking systems.

●      Comfort building relationships with a wide range of partners — from caseworkers and caregivers to school staff, community organizations, and donors.

●      Completion of, or willingness to complete, TBRI® Caregiver Series training (required for all Jacob Center staff).

●      Valid driver’s license for a minimum of 2 years, clean driving record, and reliable transportation; this role includes regional travel.

●      Ability to pass CBI/FBI/Trails/Colorado Courts background checks and UA drug screening, post contingent position offer.

●      Flexibility in work hours to accommodate community events and family schedules; may include evenings and weekends.

●      Strong commitment to advancing racial equity through culturally responsive practices. Must demonstrate alignment with agency values, policies, and commitment to racial equity and anti-racist work.

PREFERRED QUALIFICATIONS:

●      Bilingual (Spanish/English).

●      Familiarity with the child welfare, foster care, juvenile justice, or housing instability service systems.

YOU'LL THRIVE HERE IF YOU:

●      Enjoy being out in the community rather than behind a desk all day.

●      Like connecting people to resources and following through until the connection actually happens.

●      Can hold both the relational side of the work (meeting people where they are) and the administrative side (accurate records, timely reporting).

●      Move comfortably between big-picture partnership building and hands-on, one-on-one family coaching.

●      Bring energy and organization to juggling outreach, partnerships, and direct family support all at once.

OUR COMMITMENT TO YOU:

●      Full training in TBRI® Caregiver Series as part of onboarding.

●      Supportive team culture across programs and departments.

●      Mileage reimbursement for community travel.

●      The autonomy to manage your own outreach calendar and community relationships.

●      A team culture that values transparency, equity, and felt safety.

PHYSICAL DEMANDS AND WORK ENVIRONMENT:

●      Ability to use computerized client database systems for documentation and record keeping.

●      Ability to communicate with clients, families, partners, and other professionals in person, by telephone, and by email.

●      This position requires regular travel throughout Larimer County. Employees must maintain a valid driver’s license, proof of insurance, and reliable access to a personal vehicle for work-related duties.

ESSENTIAL FUNCTIONS:

●      Champion The Jacob Center’s mission and values in all professional interactions with staff, clients, community partners, and stakeholders.

●      Serve as the interagency point person for TBRI®-related programming needs, coordinating community outreach and direct family support across program areas.

COMPENSATION AND BENEFITS:

Wage: $25-$27/hour, depending on experience

Benefits:

●      Flexible PTO

●      11 Paid Annual Holidays

●      Group Health and Life Insurance (eligible after 60 days full-time employment; Jacob Center contributes to monthly health insurance premiums)

●      EAP reimbursement

●      Paid professional development and training opportunities

●      Mileage reimbursement for community travel

HOW TO APPLY:

Submit the following by 10/21/2026 at 1pm MT on LinkedIn or to adrienne.birt@jacobcenter.org.

●      Resume/CV

●      Cover letter describing your experience with community outreach or family-facing work, and what unique perspective you’d bring to this role

●      Two to three professional references
