Position: NY Connects Education and Outreach Coordinator

Salary: $66,000


General Description:


The NY Connects Program provides Information and Assistance and Options Counseling/Person-Centered Counseling on Long Term Services and Support (LTSS) for people with disabilities and older adults. The Outreach & Education Coordinator position requires experience establishing community connections for outreach, public speaking, and coordinating events. The Outreach & Education Coordinator will also often be required to travel between boroughs and should excel at organizing their time and schedule. Management experience strongly preferred.


Reports to:  Director of NY Connects


Duties and Responsibilities: 


• Responsible for in-person, off-site, and online educational workshops, and events to help community partners, consumers, and others with disabilities understand the various Long-Term Services and Supports (LTSS) and community services available to them.

• Create and implement organizational charts and Excel spreadsheets tracking outreach data and providing timely and complete performance reports to supervisor.

• Ensure the implementation of the State’s Program Standards related to outreach and education.

• Confer with the NY Connects Program Director to review the performance of the outreach unit regarding strategies for overcoming barriers to meet goals and objectives.  

• Link with content experts in the field, community-based organizations, government agencies, coalitions, and other partners to ensure counselors can provide accurate information, appropriate referrals, and options counseling. 

• Meet with disability-services agencies, health care providers, hospital social workers, home and community-based service providers, and other agencies for cultivating cross-referral partnerships to NY Connects.

• Assist in the development of educational and outreach materials.

• Create and maintain a calendar of workshops/training and a calendar for outreach events.

• Manage and supervise outreach staff (approximately 3 to 5 outreach specialists) including assisting them with the planning and execution of outreach events and outreach plans, work with partner organization’s outreach staff.

• Oversee the Outreach staff training and contribute to outreach training curriculum; collaborate with borough managers to provide training for benefits counselors’ and managers skill gaps.

• Participate in New York Connects management team, formulate, and administer project policies and procedures, develop, and implement a workplan that ensures timely completion of short-term and long-term goals and objectives to meet contract requirements.

• Contribute listings and information to the NY Connects Resource Directory through agency linkages created with workshops and community outreaches.

• Collaborate with the Communications team to provide NY Connects material and content for CIDNY’s website, Facebook, Twitter, and other electronic media outreach to CIDNY consumers and others in the disability community

• Order and maintain a supply of materials for use by outreach workers.  Maintain an inventory of materials for outreach for reorders.

• Required to travel in all five boroughs for outreach and other events.

• This position requires occasional attendance at outreach events on nights and weekends.

• Attend agency in-service training and unit meetings, as well as any other agency-related activities.

• Perform other duties as assigned.


Education and/or experience required:

• BA or BS degree required; MA preferred

• Multilingual preferred

• Ability to travel 

• Excellent research, writing, computer literacy, social media literacy, organization, and communication skills are required

• Experience in providing workshops and community outreach required

• Knowledge of long-term services and supports and public benefits

• Prior management experience strongly preferred

• Must be computer literate on Microsoft Programs; ability to quickly learn and use other databases/technology is helpful

• Respectful and comfortable with persons from different backgrounds

• Understanding or experience with disability-related issues


Salary:  $66,000 + Excellent Benefits Package
